This agreement governs participation in any Physicians Committee for Responsible Medicine (“Physicians Committee”) class, conference, event, meeting, seminar, or other program (collectively “the Program”). By registering for the Program you, or any registrant on whose behalf you register, agree to these terms, which form a binding legal contract between the Physicians Committee and you or the participant(s) on whose behalf you register. If you register on behalf of another participant, you warrant that you have the authority to bind that participant and to enter into this agreement on behalf of that participant.

Any information presented or exchanged throughout the Program is not intended to, and does not, create a physician-patient relationship or substitute for professional medical advice, diagnosis, or treatment. The information presented in the Program is not intended to replace the advice of your doctor or other health care professional with whom you consult. Consult with your doctor or other health care professional regarding any medical condition.
Confidentiality
Some Programs, such as group classes, may involve the voluntary disclosure of personal or health information by participants. That information is private and confidential. You agree to respect the confidentiality of the personal or health information shared by participants, and you agree that you will not share that information with other parties. If your participation occurs via remote conferencing, you agree to participate only in a private setting in which the confidential information of participants cannot be overheard or seen by third parties. You agree not to photograph or record the Program. You confirm that you knowingly and willing participate in the Program.
Recording
You acknowledge that the Physicians Committee may record the Program, and you consent to the recording and any use of your voice, name, and/or likeness in any future uses of the recording.
Limitation of Liability
You release and discharge the Physicians Committee and its subsidiaries, directors, employees, contractors, or agents from any and all liability, claims, or demands due to unauthorized publication by other participants of personal or health information you share during the Program or arising out of any other aspect of your participation in the Program.
